At SIECUS, Smith is one of the nation's leading advocates on issues related to reproductive and sexual health including sexuality education, teen sexual health, HIV/AIDS, and sexual orientation. Under his guidance, SIECUS has become a powerful force behind federal, state, and local efforts to promote comprehensive education about sexuality and to combat ineffective and potentially harmful approaches such as abstinence-only-until-marriage programs. Smith is one of the founders of the Caucus for Evidence-Based Prevention of which the San Francisco AIDS Foundation is an active member.
